Description

Since I've made the last round of changes to the system, I believe parts of the PDF are broken. Checkouts need to be added to the pdf functionality, and the other fields should be verified to still work properly. The form to create the pdf could also have a nicer time-chooser instead of the text field.

I believe I fixed everything up. Content-wise, it should be solid. General appearance could still use some work... My thought is to make sure this is a "requirement" before investing more time into it
